Transition Support

As young people with cerebral palsy transition into adulthood, they may begin to experience new challenges such as fatigue, pain, stiffness or changes in mobility. I provide physiotherapy support to help young people manage these changes with confidence through personalised stretches, strengthening programmes and practical strategies for everyday life.


A key focus is helping young people build a personalised self-management toolkit they can take forward into adult life — giving them the knowledge, routines and confidence to better understand their body, manage symptoms and maintain their physical wellbeing independently.


Support can also include helping young people adjust to college, university, apprenticeships or employment, with a focus on promoting independence, wellbeing and long-term physical health.
